Yes, it's happened to me several times. First, I don't care if someone gets fixated on my work. We get paid by the read here on Medium, so if someone wants to come to my stories and insult me...whatever, more money for me, LOL.

I can usually gauge whether a commenter has a genuine interest in a discussion or just wants to fight or throw insults around. If they just have a different point of view and are respectful, I'll engage and see where it goes.

If not, I sometimes just give a one line reply, usually something smart-assed because that's just me and I leave it there. It's not up to me to stop people from making themselves look stupid and their comments often do just that. :)
